There are two possibilities:

1. Your voice mail password was lost on your cell phone.
At very beginning, you were asked to set a password of your voicemail. That password is recorded in ATT service database as well as your cell phone. Every time when you get a new phone or update your phone with firmware or iOS4, for example, your cell phone may lost that password. All you have to do is to set voicemail password on your phone again. No need to call ATT
2. ATT service was in trouble with all your record. In this way, you have to reset your voicemail password as you did at very very beginning with ATT service as well as your cell phone.
Many times ATT service people screwed up your record at the time you called. In this way, best thing they should do is to deleted all your service record and re-create new. That will require a approving from ATT service manager. So when you call ATT service next time, you may want to speak with a manager first.

Similar problem. My voice mail kept asking for a password, nothing worked. Called AT&T. They acted as if they knew about a voice mail problem. Had to reset my voice mail several times before it started working normally again. Suddenly all these older voice mails popped up.
Wish AT&T would fess up when there is a problem with anything. I had several voice mails from work that were a few days old I had no clue about.

By the way, when I was on the phone with AT&T, I was on the iPhone 4 and had a dropped call. AT&T called back immediately and asked if I hung up. I said no - dropped call! They asked if I were in a "structure". I asked "what do you mean - I'm in my house" They said "any structure - they do not guarantee service in any structure, including a house". At that point, I lost it with them.

Same thing happened to me, but with my 3GS. Apparently I didn't notice until folks started sending me emails to let me know my voice mail was full!! Apparently the problem happened to me at some point earlier this year.

I spent several hours on the phone with ATT, entered several different character strings, had them delete and recreate my account.
Nada. They kept bumping me up, and no one had any idea. So I took it to the local store and spent a good bit of time their, where they
suggested I take it to an apple store and have *them* look at it.

So I did. The "genius" at the genius bar says the phone is fine, that it is some problem with ATT!!!

Just wanted to let you know you weren't the *only* one.

If it any consolation, you can still access and set up your voice mail manually, most likely, as I was able to do so.
Just go to the keypad and press the "1" key for several seconds and it will go to voice mail, just like it would
on a basic cell phone.

So, in my case, the phone company *and* Apple are each pointing fingers at the other, because of course
it couldn't possible be a fault with the iPhone (apple sez) or with the ATT network (att says) ARGH!!

Hang in there! And read my itunes adventure for even more amusement! LOL

I mean i have no voicemail at all! The screen on the phone where the voicemail would be had the message that it needed to be setup. After entering the password ( both the old one and several new ones ) it would prompt me to choose default or custom greeting then when I would try to save it would sit there trying to connect then eventually give a popup that it couldn't connect to voicemail and to try again later. The tech had me try about a dozen different things including several different passwords, a restore, turned my voicemail off then on and I bet I had to turn the phone off the on about 15 (!!) times sovthey could reset the account or frequency or who knows what. Before all thus I could at least dial and hold "1" to get to my voicemail, or simply dial my own number and do it the 'old fashion' way but now they have it really messed up and I get some strange message asking me to enter a ph number and a mailbox number to connect. Then nothing will get
me past that prompt. The tech actually admitted to me that they had no info onthe iPhone 4 yet and were using the 3gs flowcharts for troubleshooting!! Again, now both my phones on the same account have no texting ability, the iPhone 4 has no text voicemail of any kind and not edge/3g data connection at all and they want me to wait till at least the 30th before they will get back to me with a possible solution! It's a very frustrating mess because I use my phone an awful lot on my job and a few days is a real pita! But what do ya do? Anyway thanks for listening!!
Oh yeah, the ph number is one that was on an iPhone already when I switched it to the new phone and I have had the number for almost 10 years through a few different carriers and giving it up shouldn't have to an option IMO.
