If you do a Restore As New your folders will be gone. Your apps will be reinstalled on your iPhone in alphabetical order without folders.
If you do a Restore As New, any app which saves data to your iPhone but which does not get synced to your computer or which does not provide some other means of backing up the data will lose it's data. Be sure to check EVERY app you have on your iPhone to see if it has any data you want/need, and that there is some method of saving that data if you really want it, before doing a Restore As New.
