I can answer the first question but not sure of the second.

If you change the file XYZ in my documents, the change will not also
be sent to the iDisk XYZ file. If you want that to happen, I would
suggest only having the file on iDisk (keeping a local copy of the
iDisk available so you can access when not connected to the Internet
and also enabling automatically sync). Then when you update the file,
you access it on your computer from iDisk instead of My Documents.
If you want to access it from both, what I would recommend is that
you create an alias of the files you are storing on iDisk and put
those aliases in My Documents.

Not sure how the changes on iPad get propogated through.
