That happened to me.
First thing to do is call AT&T because that's part of the
provisioning. For no reason, several account options changed (texting
was also off). They fixed all that and still no go. Eventually, I had
to reset the phone and start over. AT&T said the most common cause is
the attempt to use non-kosher provisions (specifically, tethering)
but that wasn't true in my case.
