Settings > General > Location Services > Camera (On/Off) - if you turn it on
in there, it stays on permanently unless you turn it off. If it you turn it
off, you are prompted every time you open the camera app to allow it to use
Location Services (AKA GPS) or not.

AT&T doesn't support the phone to do this, AT&T does of course sell a
carrier level "feature" for something like $2.99 or $4.99 per month where
you can block numbers or you can set the caller's number as a contact and
assign the ring tone as silent.
